Here are the fees for issuing medical certificates.
Issued for injuries requiring less than three weeks of treatment.
Issued for injuries requiring three weeks or more of treatment.
The doctor writes it based on the combined results of the examination and tests.
Your medical record is copied onto paper. This rate applies to pages 1 through 5.
This rate applies from the sixth page onward.
Confirms your outpatient visit dates and that treatment was provided.
Documents are issued in person only; they cannot be issued over the phone.
Documents are issued to the patient only; collection by proxy is not possible.
